Vaccine Exemptions by State
Parents can file religious or philosophical belief exemptions, in all but 5 states.
Below are links for each state's exemption forms, for parents to file with their state health departments for their child's school, daycare or activities. Note: medical exemptions can also be requested in all 50 states.
In addition, most states have online vaccine tracking databases. Parents can opt-out so their children are not tracked in that system.
It is important that you file for, are granted and receive your exemption certificate in hand before opting-out of your state's vaccine tracking system.
